Abstract: ［Objective］To explore the cinical efficacy of double-tunnel isometric resconstruction of medial patellofemoral ligament combined with tibial tubercle osteomy transfer for the treatment of recurrent patellar dislocation.［Methods］A retrospective review was carried among 20 patients who received arthroscpic double-tunnel isometric resconstruction of MPFL combined with tibial tubercle osteomy transfer for recurrent patellar dislocation in our hospital from May 2017 to May 2022.The socres of Lysholm、Kujala、International Knee Documentation Committee（IKDC）、knee ROM、patellofemoral angle（PCA）、patella title angle（PTA）、tibial tubercle trochlear groove distance（TT-TG）、 quadricep angle（Q angle）were compared between before and after operation.MRI used to evaluate patellofemoral cartilage thickness and bone marrow edema due to cartilage injury.And whether it had happened Patellar redislocation within 12 months..［Result］The preoperative socres of Lysholm、Kujala、IKDC were significantly lower than after operation 12 months （all p＜0.01）；The knee ROM was similar with before operation（p＞0.05）；The preoperative PCA、PTA、TT-TG and Q angle were higher than after operation 6 months（all p＜0.01）.The thickness of patellofemoral joint cartilage changed before and 12 months after surgery (p＞0.05). Twenty patients had 24 patellar and trochlear bone marrow edema before surgery and 8 bone marrow edema 12 months after surgery.No dislocation occurred in the patients after 12 months.［conclusion］Double-tunnel isometric resconstruction of MPFL combined with tibial tubercle osteomy transfer have achieved satisfactory clinical effect in the treatment of recurrent patellar dislocation.
