    目的 研究人工全髋关节置换术后无菌性松动局部组织表现与CXCL16的表达水平及其临床意义。方法 以2017年6月~2023年6月44例无菌性假体松动行翻修的患者为实验组,44例同期于我院行初次全髋关节置换术的患者为对照组。取两组患者界膜组织,采用HE染色法观察形态学改变,采用免疫组化检测实CXCL16的表达情况。结果 HE染色结果显示实验组假体界膜组织中有较多多核巨细胞浸润,同时组织学评分结果显示,两组组织学评分之间存在显著差异(P<0.01)。免疫组化染色结果显示,实验组患者界膜组织中CXCL16的表达水平显著高于对照组(2=55.942,P<0.01)。分层分析结果显示,组织学评分与不同假体类型之间存在统计学意义(P<0.01),CXCL16的OD值与不同假体类型、假体生存期之间均存在统计学意义(P<0.05)。相关性分析结果显示,组织学评分与假体生存期之间存在相关性(r=0.383,P<0.05),CXCL16的表达水平与CXCL16的OD值、假体生存期、翻修前Harris评分之间均存在相关性(P<0.05)。结论 人工髋关节置换术后无菌性松动患者假体界膜组织中CXCL16的表达水平显著高于对照组,推测该因子极大可能参与了无菌性松动的发病过程。

    Abstract:

    Objective To investigate the expression level of CXCL16 and its clinical significance in the local tissue manifestations of aseptic loosening after total hip arthroplasty. Methods A total of 44 patients who underwent revision of aseptic prosthesis loosening from June 2017 to June 2023 were selected as the experimental group, and 44 patients who underwent primary total hip arthroplasty in our hospital during the same period were selected as the control group. HE staining was used to observe the morphological changes and immunohistochemistry was used to detect the expression of real CXCL16. Results HE staining results showed that there were more multinucleated giant cells infiltrated in the border membrane tissue of the prosthesis in the experimental group, while the histological scoring results showed that there was a significant difference between the histological scores of the two groups (P < 0.01). Immunohistochemical staining showed that the expression level of CXCL16 in the perimembranous tissues of patients in the experimental group was significantly higher than that in the control group (2 = 55.942, P < 0.01). The results of stratified analysis showed that there was statistical significance between histologic scores and different prosthesis types (P < 0.01), and between OD values of CXCL16 and different prosthesis types and prosthesis survival (P < 0.05). The results of correlation analysis showed that there was a correlation between histologic score and prosthesis survival (r = 0.383, P < 0.05), and there was a correlation between the expression level of CXCL16 and the OD value of CXCL16, prosthesis survival, and pre-revision Harris score (P < 0.05). Conclusion The expression level of CXCL16 in the prosthesis border membrane tissue of patients with aseptic loosening after artificial hip arthroplasty was significantly higher than that of the control group, and it is hypothesized that this factor is greatly likely to be involved in the pathogenesis of aseptic loosening.
