[Objective]To investigate the clinical efficacy of platelet-rich plasma lysate combined with Jiegu Pill in the treatment of nonunion of tibia and fibula. [Methods]From Januaryuary 2019 to Januaryuary 2022,46 cases of tibiofibular nonunion were treated in our hospital,including 34 males and 8 females,aged 45-62 years (median 53.5 years);Forty-six patients were randomly divided into platelet-rich plasma lysate group (control group,) and platelet-rich plasma lysate combined with Jiegu Pill Group (treatment group,).The age,sex and course of disease were recorded.In both groups, 100 ml of venous blood was drawn from the elbow vein to prepare platelet-rich plasma.The lysates of platelet-rich plasma were prepared by thawing and centrifugation.The components and contents of lysates of platelet-rich plasma were determined.In the treatment group,the platelet-rich plasma lysate combined with Jiegu pill was given orally Jiegu pill for one month without taking any other drugs.The Healing of fracture was evaluated on X-ray before and 2,3,4,6 months after treatment by callus and fracture line imaging scoring system,observe the occurrence of complications.[Results](1)Detection of platelet-rich plasma cytokines. The levels of cytokines VEGF and TGF-β were (583.87±23.51)pg·ml-1 and (195.73±26.08)pg·ml-1.(2)Image evaluation.All patients were followed up for a median of 4 months (range,3-6 months).Before treatment,there was no significant difference in callus imaging score between the two groups(P > 0.05) Two months after treatment,three months, four months and six months after treatment,the callus imaging scores of the treatment group were higher than those of the control group.There was significant difference in callus imaging score between the two groups (P < 0.05).Before treatment,there was no significant difference in fracture line imaging score between the two groups (P > 0.05).Two months after treatment,three months after treatment,four months after treatment and six months after treatment,the scores of fracture line in the treatment group were higher than those in the control group,there was significant difference between the two groups in fracture line imaging score (P < 0.05).Forty-four patients achieved bone union.The time of nonunion union was 3-6 months (median 3.5 months).All patients had no vascular and nerve injury and other complications. [Conclusion]The treatment of nonunion of tibia and fibula with platelet-rich plasma lysate combined with Jiegu pill is effective and has a good application prospect.
