Abstract:Backgraoud:The perception that broken anterior cruciate ligament (ACL) cannot heal is changing with the improvement of basic biology in recent years. There has been a renewed interest in acute ACL repairs with newer internal brace technique. Objective:The aim of this study is to provide an overview of the current evidence presenting outcomes of exploring the histological characteristics of ACL repair with internal brace technique and comparing its therapeutic and prognostic results with the ACL reconstruction。 Methods:The biomechanical tests, histological tests, animal experiments, and clinical studies on ACL repair with internal brace technique in decade were reviewed, the effectiveness and practicability of this technology were analyzed and summarized. Results:Internal Brace technique can provide a stable environment for ACL tissue healing. In the biomechanical research, internal brace technique alleviates the relaxation and micromotion caused by tension by sharing the load of ACL after repair. Meanwhile, it can reduce postoperative pain and complications and shorten the rehabilitation period compared with ACL reconstruction. But ,the danger of the procedure must is highlighted if strict patient selection is not appreciated. Conclusion:ACL repair with internal brace technique is a promising treatment for acute proximal ACL tear.It not only can almost achieve equivalent efficacy to standard ACLR but also reduce the pain and speed up the rehabilitation. However, there are limitations in age, level of sports participation and timing of treating while patient be selected.ACL repairing with internal brace technique supports The ideal patients who are non-high-level athletes over 25 years of age with acute proximal ACL tear.
