Abstract:To introduce the surgical technique and preliminary clinical results of Herbert screw via Ganz approach for fixation of femoral head fracture. [Methods] Between Jaunary 2015 and September 2021, 10 patients with an average follow-up of more than 6 months underwent Herbert screw via Ganz approach for fixation of femoral head fracture. The operating method is using an osteotomy lengthways from the upper vertex of the greater trochanter to the Posterior edge of the lateral femoral muscle to revealing the joint capsule. Then cut open the joint capsule with the shape of "Z" along the long axis of the femoral neck, dislocate the femoral head to anterolateral side, debride the articular cavity of hip joint, reduce femoral neck fracture block with 2-3 Herbert screws, reduces the femoral head by internal rotation and restore bone block of greater trochanter with 2 cortical bone screws. [Results] All patients underwent successful operation with operation time of 139.5±30.5(100-195)min, blood loss of 310.0±213.2(100-700)ml. The patients got clinical fracture healing in 3.6 months (3-6 months). The hip function scored 6 months after surgery according to Epstein-Thompson classification with the following results: 5 cases had excellent results, 4 cases were rated as good,while 1 as fair. [Conclusion] Herbert screw via Ganz approach for fixation of femoral head fracture with low complication rate and good effect, can fully expose the acetabulum and femoral head, can reduce the fracture end under direct vision.
