Objetctive To investigate the treatment and effectiveness of anterior cruciate ligament injury with complete-body radial tears of the lateral meniscus. Method From January 1, 2019 to December 30, 2020, we studied 14 patients who were treated by autograft I.D.E.A.L femoral tunnel position placement for anterior cruciate ligament reconstruction, and the body radial tears of the lateral meniscus repair. The subjects comprised 11 males and 3 females ranging in age from 18 to 56 (mean 35.7) years, Left knee 5 cases, right knee 9 cases. Causes of injury include 11 cases of sports injury, 2 cases of traffic accidents, 1 case of daily life injury. Preoperative X-ray and MRI examinations were performed. Lachman test was positive in all cases. The incidence of anterior cruciate ligament injury with lateral meniscal radial tear were diagnosed and treated by arthroscopy. preoperative International Knee Documentation Committee (IKDC) scores and Lysholm score was ( 46.5 ± 5.5 ) ,( 43.3 ± 4.5 ) respectively. the mean interval from injury to initial arthroscopy was 5.2 says (from 3 to 15 days). Results Duration of surgery was 70 – 100 min, with an average of 82.7 min. All incisions were healed, and there were no infection , neurovascular injury . All patients were followed up for 12 – 24 months, with an average of 16.8 months. Three patients had lower limb intermuscular venous thrombosis within 7 days after operation, and the condition was stable after treatment. At the final follow-up, lachman test was negative and the knee joint activity returned to normal range. postoperative IKDC scores and Lysholm score was (94.67±3.67 ) ,( 90.37±4.49 ) respectively. analysis indicated that scores in both groups improved from pre- to postoperatively（t= ?12.386，P=0.000；t=?22.861，P=0.000）. Only one patient reported the pain of tenderness of lateral joint clearances after 6 month of the operation, MRI shows that meniscus was not healed, partially meniscectomy of white zone under arthroscopic relieve the pain. Conclusion Satisfactory results are achieved when autograft using for I.D.E.A.L femoral tunnel position placement anterior cruciate ligament reconstruction , with horizontal and Vertical Mattress Suture for the body radial tears of the lateral meniscus repair.
