    目的:观察采用经颅直流电刺激(tDCS)联合核心稳定性训练(CSE)治疗慢性非特异性下背痛(CNLBP)的临床疗效。 方法:对慢性非特异性下背痛患者进行前瞻性研究,共选取64名符合纳入排除标准的患者,随机分成试验组(n=32)和对照组(n=32)。对照组通过常规理疗及核心稳定训练进行治疗,试验组采用经颅直流电刺激联合核心稳定性训练进行治疗。通过两组患者治疗前及治疗后8周的视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)、姿势稳定测试等指标的比较,分别评估患者的疼痛程度、功能障碍程度及姿势控制能力。 结果: 试验组治疗8周后的VAS评分、ODI指数评分、总体稳定指数、前后稳定指数及左右稳定指数方面较治疗前均有显著下降(P<0.05)。对照组治疗后VAS评分、ODI指数评分较治疗前明显下降(P<0.05),稳定指数较治疗前后差异无统计学意义(P>0.05)。治疗后的两组结果比较,试验组在VAS评分、ODI指数、总体稳定指数、前后稳定指数及左右稳定指数方面均明显小于对照组(P<0.05)。 结论:经颅直流电刺激联合核心稳定性训练能有效缓解慢性非特异性下背痛患者的疼痛症状,改善功能障碍及姿势控制能力。

    Abstract:

    Objective To observe the clinical effect of transcranial direct current stimulation (tDCS) combined with core stabilization exercises (CSE) on chronic non-specific low back pain (CNLBP). Methods 64 patients with CNLBP were randomly assigned to experimental group (n= 32) and control group (n = 32). Both groups received core stabilization exercises and routine physical therapy. The experimental group accepted tDCS. Visual Analogue Scale (VAS), Oswestry Disability Index (ODI) were used to evaluate the clinical effect. The postural stability was evaluated by the Biodex Balance System. Postural stability indices included overall stability index (OSI), anterior-posterior stability index (APSI) and medial-lateral stability index (MLSI). Results After treatment, VAS, ODI scores and stability index were significantly decreased in experimental group (P<0.05). VAS, ODI scores were significantly decreased in control group (P<0.05). There was no significant differences in stability index (P>0.05). In experimental group, VAS, ODI score, OSI, APSI and MLSI were significantly lower than control group after treatment (P<0.05). Conclusion tDCS combined with CSE can alleviate the pain of CNLBP patients and improve clinical efficacy and dynamic posture control.
