    目的 介绍镜下“内夹板”技术(近端套索缝合联合外排锚钉缝线桥技术)修复急性闭合性跟腱断裂手术技术及2年初步临床应用效果。 方法 2018年6月-2019年1月采用上述技术治疗并获随访大于2年急性闭合性跟腱断裂19例。在关节镜监视下使用2#高强度缝线套索缝合跟腱断端近侧内外侧,将缝线尾端经腱周膜深面引导至跟骨结节两侧,使用直径4.5mm的外排锚钉固定缝线尾端,逐步收紧缝线并使用B超监视跟腱断端对合良好后拧紧固定。结果 19例患者均顺利完成手术。无腓肠神经损伤、感染及再断裂;1例因锚钉刺激需二次干预;末次随访AOFAS评分96.11±5.02,ATRS评分96.89±4.17,VAS评分0.39±0.50;术后6个月恢复伤前运动水平,末次MRI显示跟腱塑形良好。 结论 镜下“内夹板”技术是一种可靠、有效的急性闭合性跟腱断裂治疗方法。

    Abstract:

    Objective To introduce arthroscopic "internal splinting" repair technique (arthroscopic locking loop stitch with knotless anchor suture-bridge technique) for treatment of acute Achilles tendon rupture and report the 2-year preliminary clinical outcomes. Methods From June 2018 to January 2019, nineteen patients with acute closed Achilles tendon rupture were treated with arthroscopic "internal splinting" repair technique and follow-up more than 2 years. Under arthroscopic monitoring, 2# high-intensity sutures were used to suture the medial and lateral proximal of the Achilles tendon stump. The ends of the sutures were guided to both sides of the calcaneal tuberosity through the deep surface of the Achilles tendon. The ends of the sutures were fixed using 4.5 mm knotless anchor, the sutures were gradually tightened and fixed after the Achilles tendon stump was well aligned using B-ultrasound monitoring. Results All 19 patients completed the operation successfully. No gastrocnemius nerve injury, infection or re-rupture was observed. One patient removed the anchor because of irritation at the medial knotless anchor. At the last follow-up, the average AOFAS score was 96.11 ± 5.02, the average ATRS score was 96.89 ± 4.17, and the average VAS score was 0.39 ± 0.50. All patients returned to their pre-injury exercise level at 6 months after surgery. MRI showed that the injuried tendons restored continuity. Conclusion Arthroscopic “internal splinting” repair technique is a reliable and effective treatment for acute closed Achilles tendon rupture.
