    摘要:[目的]探讨II型糖尿病骨代谢异常与认知障碍的相关性。[方法]收集2020年7月至2021年9月于山东第一医学大学第二附属医院就诊的II型糖尿病(type II diabetes mellitus,T2DM)患者45例。包括患者的一般临床资料、血清骨钙素(osteocalcin,OC)、糖化血红蛋白(hemoglobin A1c, HbA1c)、骨密度(bone?mineral?density,BMD),评估每一位患者的认知水平。参照骨密度进行分组,比较组间患者的认知改变,分析认知与临床相关指标、BMD与OC之间的相关性。[结果]组间MoCA评分、BMD存在差异统计学意义(P<0.05),T2DM骨质疏松组低于其他两组。BMD与MoCA评分呈强正相关(r=0.628,P<0.001),OC与BMD呈显著正相关性(r=0.733,P<0.001)。骨密度能单独解释对认知影响的38.1%;OC能单独解释对骨密度影响的52.7%。[结论]认知障碍随着II型糖尿病骨代谢异常的进展而逐渐加重。

    Abstract:

    Abstract:[Objective]To investigate the correlation between abnormal bone metabolism and cognitive impairment in type II diabetes mellitus(T2DM).[Materials and Methods]A total of 45 patients with T2DM who were admitted to the Second Affiliated Hospital of Shandong First Medical University from July 2020 to December 2021 were collected. The general clinical data, serum osteocalcin (OC),hemoglobin A1c(HbA1c) and the bone mineral density(BMD) were collected, evaluate the cognition of each patient. The subjects were grouped by the reference of BMD,the cognitive changes of patients between groups were compared, and the correlation between cognition and clinical related indexes, bone mineral density and osteocalcin were analyzed..[Results]The difference in MOCA scores and BMD between groups was statistically significant(P<0.05). The T2DM with osteoporosis group was lower than the other two groups,BMD and MOCA scores showed strongly positive correlation(r=0.628,P<0.001),OC was positively correlated with BMD(r=0.733,P<0.001), BMD alone explained 38.1% of the effect on cognition;OC alone explained 52.7% of the effect on BMD.[Conclusion]Cognitive impairment is gradually aggravated with the progression of abnormal bone metabolism in T2DM.
