    摘要:

    [目的]介绍一种新型经皮腰椎融合术的手术技术和初步临床结果。[方法]2017年12月~2019年02月,23例腰椎峡部裂性滑脱症患者接受上述手术治疗。C臂透视下经皮椎弓根穿刺并置入导丝,后正中线旁开6~9cm作为进针点滑脱节段椎间隙穿刺,导丝引导下椎间孔扩大成形,置入工作套管,内镜下行侧隐窝与椎间孔扩大减压;接着导丝引导下自制器械逐级撑开椎间隙。然后,沿导丝置入椎弓根螺钉和连接棒行滑脱椎体提拉复位。通过自制工作通道放入铰刀和刮匙清理椎间隙和终板,镜下清理干净后,植入自体骨(或同种异体骨)和椎间融合器。调整紧固钉棒系统,完成固定。[结果]所有患者手术均顺利完成,无严重并发症发生。末次随访时,腰痛和腿痛VAS评分、ODI评分、椎间隙高度、患椎滑脱距离均较术前明显改善(P<0.05),CT显示椎间植骨均骨性愈合。[结论]采用新型经皮腰椎融合术治疗腰椎峡部裂性滑脱症的手术技术可行,符合目前脊柱外科微创发展方向,是一种安全、有效的微创手术方式。

    Abstract:

    [Objective] to introduce a new surgical technique and preliminary clinical results of percutaneous lumbar fusion Methods] from December 2017 to February 2019, 23 patients with lumbar spondylolysis spondylolisthesis underwent the above surgical treatment. Percutaneous pedicle puncture under C-arm fluoroscopy and placement of guide wire, 6 ~ 9cm beside the posterior midline as the needle entry point, sliding off segment intervertebral space puncture, expansion and shaping of intervertebral foramen under the guidance of guide wire, placement of working cannula, enlargement and decompression of lateral recess and intervertebral foramen under endoscopy; Then, under the guidance of guide wire, self-made instruments were used to open the intervertebral space step by step. Then, pedicle screws and connecting rods were inserted along the guide wire for lifting and reduction of spondylolisthesis vertebral body. Put reamer and curette into self-made working channel to clean intervertebral space and endplate. After cleaning under microscope, implant autologous bone (or allogeneic bone) and intervertebral fusion cage. Adjust the fastening nail bar system to complete the fixation Results] all the operations were successfully completed without serious complications. At the last follow-up, VAS score, ODI score, intervertebral space height and spondylolisthesis distance of low back pain and leg pain were significantly improved compared with those before operation (P Conclusion] the new percutaneous lumbar fusion technique for the treatment of lumbar spondylolysis spondylolisthesis is feasible, in line with the current minimally invasive development direction of spinal surgery, and is a safe and effective minimally invasive operation.
