    摘要:[目的] 探讨皮质骨轨迹(CBT)椎弓根钉在腰椎AO spine C型骨折翻修术中的应用优势和临床疗效。[方法] 回顾性分析应用皮质骨轨迹(CBT)椎弓根钉治疗腰椎骨折翻修术的23例患者临床资料,记录二次手术操作时间、术中出血量和手术并发症。应用视觉模拟评分法(VAS)、Oswestry功能障碍指数(ODI)问卷表和日本骨科协会(JOA)腰椎功能评分对患者翻修术后功能改善情况及临床疗效进行评价。[结果] 23例患者手术时间平均为(158.55±36.71)min;术中出血量平均为(222.71±51.34)ml。1例术中并发脑脊液渗漏。所有患者随访平均(17.86±4.63)月。腰痛及腿痛术前VAS评分、ODI评分、JOA评分与末次随访各自评分相比,二者差异均有统计学意义。所有患者腰腿痛症状均获得缓解,躯体功能改善满意。[结论] 皮质骨轨迹(CBT)椎弓根钉应用于腰椎AO spine C型骨折翻修术有其特有的技术优势,获得了良好术后效果。

    Abstract:

    Abstract: [Objective] to investigate the application advantage and clinical effect of cortical bone trajectory (CBT) pedicle screw in revision of Ao spine C fracture of lumbar spine[ Methods] the clinical data of 23 patients who underwent revision lumbar fracture treatment with cortical bone trajectory (CBT) pedicle screw were analyzed retrospectively. The time of secondary operation, intraoperative bleeding and surgical complications were recorded. Visual analogue scale (VAS), Oswestry dysfunction index (ODI) questionnaire and Japanese Orthopaedic Association (JOA) lumbar function score were used to evaluate the functional improvement and clinical efficacy after revision surgery[ Results] the average operation time of 23 patients was (158.55 ± 36.71) min; The average amount of intraoperative bleeding was (222.71 ± 51.34) ml. Cerebrospinal fluid leakage occurred in 1 case. All patients were followed up for an average of (17.86 ± 4.63) months. There were significant differences in VAS score, ODI score and JOA score of low back pain and leg pain before operation compared with their respective scores in the last follow-up. The symptoms of low back and leg pain were relieved and the physical function was improved satisfactorily[ Conclusion] the application of cortical bone trajectory (CBT) pedicle screw in the revision of lumbar Ao spine type C fracture has its unique technical advantages and achieved good postoperative results.
