    目的:探讨3D打印技术以及3D打印金属垫块、金属小梁臼杯在髋关节翻修中的疗效。方法:2015年7月至2019年7月,采用3D打印模型术前规划,术中应用3D打印金属垫块、金属小梁臼杯治疗30例髋臼侧骨缺损髋关节翻修病例,评价指标包括术前术后疼痛视觉评分(VAS)、Harris髋关节功能评分、术后X线片,随访观察并发症发生情况。结果:30例患者获随访,随访时间1.5-4年,中位数2.5年。手术时间(125±18)min,术中出血量(678±32)ml,末次随访VAS评分(t=2.048,p<0.05)、Harris髋关节功能评分(t=3.496, p<0.05)均明显改善。随访期间内,未出现关节假体松动、感染,其中1例术后第3周出现髋关节前脱位,麻醉下复位后,髋关节支具外展中立位固定4周,后未再出现脱位。结论:在髋关节翻修中应用3D打印技术以及3D打印金属垫块、金属小梁臼杯,短期效果良好,并发症少,安全有效。

    Abstract:

    Abstract Objective: To evaluate the 3D printing technology, 3D printing augment and titanium trabecular acetabular cup in hip revision. Methods: From July 2015 to July 2019, 30 cases patients with acetabular bone defect underwent hip revision were treated with 3D printing augment and trabecular acetabular cup. The evaluation indexes included preoperative and postoperative Visual Analogue Scale (VAS), Harris hip score and postoperative X-ray film. Results: 30 patients were followed up for 1.5-4 years with a median of 2.5 years. The operation time was (125 ±18)min,The intraoperative blood loss was (678 ±32)ml, At the last follow-up, Visual Analogue Scale (t = 2.048, p< 0.05) and Harris hip score (t = 3.496, p < 0.05) were significantly improved. During the following-up period, there was no component loosening or periprosthetic infection. One patient developed anterior dislocation of the hip when he underwent operation 3 weeks later. After reduction under anesthesia, the hip brace was used in neutral position for 4 weeks, and no dislocation happended again. Conclusion: The application of 3D printing technology, 3D printing augment and titanium trabecular acetabular cup in hip revision has good short-term effect, less complications.
