[Methods] 22 patients with large area soft tissue defect of foot and ankle in our hospital from January 2011 to December 2019 were treated with free anterolateral thigh perforator flap (10 cases of left foot, 12 cases of right foot). [Results] All patients were followed up for 12 months. Comparing with 1 months after operation and 6 months after operation,we found the VAS score, ROM of ankle, AOFAS score and sensation of ankle were significantly improved at the last follow-up, and the aesthetic degree was 95.45%.[Conclusion] The free anterolateral thigh perforator flap has the advantage of minimally invasive in the repair of large soft tissue defect of foot and ankle, and the survival rate of the flap is high. Therefore, this kind of flap is worthy of clinical promotion and reference.
