    摘要:

    目的 探讨合并后方韧带复合体(Posterior Ligament Complex,PLC)损伤的胸腰椎骨质疏松性椎体压缩骨折再骨折的相关危险因素。方法 回顾性分析经皮椎体成形术(Percutaneous Vertebroplasty,PVP)及椎体后凸成形术(Percutaneous balloon Kyphoplasty,PKP)治疗68例合并PLC损伤的骨质疏松性椎体压缩骨折患者的相关临床资料,其中男23例,女45例,年龄61-81岁,平均70.0岁。根据术后是否出现椎体再骨折分为再骨折组(32例)和对照组(36例),记录两组患者的年龄、性别、身高、体重、椎体骨水泥量、手术方式、穿刺方式、ODI、骨密度、Cobb角等相关临床资料,对椎体再骨折的可能危险因素进行分析。结果 所有患者术后随访时间为12-24个月,两组患者在年龄、性别、身高、体重、椎体骨水泥量、ODI、骨密度等方面无统计学差异(P>0.05),但是两组患者的手术方式、穿刺方式、Cobb角存在统计学差异(P<0.05);组内统计分析,PKP术、双侧穿刺、较大的Cobb角在合并PLC损伤的胸腰椎骨质疏松性压缩骨折发生椎体术后再骨折几率小。结论 手术方式、穿刺方式、Cobb角可能是合并PLC损伤的胸腰椎骨质疏松性椎体压缩骨折术后再骨折的危险因素。

    Abstract:

    Objective To explore the risk factors of thoracolumbar osteoporotic vertebral compression fracture and re-fracture combined with posterior ligament complex injury.Methods: The clinical data of 68 cases of OVCFs with PLC injury who underwent PVP/PKP were analyzed retrospectively There were 23 males and 45 females with an average age of 70.0 years. accordance with new vertebral fractures were divided into two group:32 cases as refracture group,while the other 36cases were termed as no refracture group. Results: All patients were followed-up for above 12-24 months. There was no significant difference in age, gender, body weight, injury-factors, amount of injected bone,Oswestry disability index and bone mineral density between the two groups (P>0.05).But there was significant difference in the surgical treatment、puncture methods kyphosis Cobb angle of surgical area (P<0.05). Conclusion: Surgical treatment、puncture methods、kyphosis Cobb angle of surgical area may be correlated with recurrent new vertebral fractures of OVCFs with PLC injury .
