Abstract:[Objective] To introduce the surgical technique and short-term clinical outcomes of distal tibiofibular syndesmosis (DTS) rupture treated by bionic double-bundle elastic fixation. [Methods] From June 2017 to June 2019, 13 patients with complete DTS rupture were treated by Endobutton plate for double-bundle and double-tunnel bionic elastic fixation. First, the DTS was anatomical reduced. Then, at resting position of the ankle joint, two bone tunnels were established with the positioning guidance device from posterolateral fibula and 3.0cm above the tibiotalar joint, including one directing anteromedial superiorly and the other posteromedially. And three assembled endobutton steel systems were guided through the bone tunnel. Finally, the Ethibond seam was tied by the "211" knot method. [Results] The operation of the 13 cases were successfully completed, all patients were followed up (14.37±5.63 months), and there was no statistical difference in the ROM (Range of motion) of ankle joint between the surgical side and the healthy side (P > 0. 05), and the average AOFAS score was 88.76 ± 3.15 points, and the excellent and good rate was 92.31%(Mazur scoring system). [Conclusion] The results of elastic fixation for complete DTS injury with double beam and double tunnel were satisfactory, and the fixation device did not need to be removed before full load bearing.
