Abstract:Abstract: [Objective] This study reported the surgical technique and clinical efficacy of the combination of Philos locking plate with the lateral intertubercular sulcus anatomic plate in the treatment of proximal humeral comminuted fractures with Neer type iii and IV fractures. [Methods] Fourteen patients with proximal humerus comminuted fractures with Neer type III or IV fractures admitted from 2018 to 2020 were included in this study. [Results] All the 14 patients underwent successful surgery, with an average operation time of 110 minutes, an average blood loss of 300ml, an average hospital stay of 10 days, and no intraoperative blood vessel or nerve injury. Mean follow-up of 10 months (6-14 months) .All the 14 fractures were successfully healed. Fracture healing time (10.48±2.31) weeks, No ischemic necrosis or reduction loss of the humerus head, The cervical trunk Angle of the humerus was about (132.71±2.83) degrees in the last follow-up X-ray,VAS score 6 months after surgery was (2.0±1.4), Neer score of shoulder joint (90.38±6.29), subjective satisfaction (8±1.2). [Conclusion] Philos locking plate combined with intertuberculous sulcus lateral anatomical plate in the treatment of proximal humeral comminuted fractures with Neer type iii and IV fractures can provide effective medial support, assist in intraoperative anatomical reduction, reduce operative time, firm fixation, and reduce postoperative complications, and achieve good clinical therapeutic effects.
