Abstract:Abstact:[Objective]To compare the curative effect of internal and lateral double plate (DLP) with retrograde intramedullary nail combined with plate (RNP) during the treatment of nonunion of the distal femur. [Methods]In the retrospective study, 29 patients who received treatment for nonunion of distal femoral at the Second Hospital of Shanxi Medical University from January 2015 to September 2018 were collected and divided into DLP group and RNP group according to different treatment methods. Among them, 15 patients were treated with DLP and 14 patients were treated with RNP. The therapeutic effects of various surgical treatment methods are analyzed and compared. [Results] All operations were successful and and there's no serious vascular complications and nerves injuries during the surgery. The DLP group shows obvious superiorities in terms of hospitalization time, postoperative drainage volum, incision length and frequency of X-rays, with significant difference(P<0.05)But there is no statistical singnificance for operative time and intraoperative blood loss between the two groups (P>0.05).29 patients were followed up for 10 to 31 months (average 18.31±5.01 months) postoperatively Both groups's pains were reduced and the function of their lower limbs had been improved postoperatively. Compared with preoperative values, HSS knee function scores of the two groups increased significantly and shortening and varus deformity of involved limb improved obviously at the last time of follow-up,with a significant difference(P<0.05) . But the improvement of knee motion function (ROM) is not obvious(P>0.05). After the surgery, the RNP group is distinctly better than the DLP group in terms of duration of walking, full weight bearing activities and fracture healing. But in the aspect of stretch or flexion of knee jont ROM, the DLP group is obviously better the the RNP group,with a significant difference(P<0.05). However, there was no statistical difference between the two groups in terms of shortening deformity, varus deformity and HSS knee function scores (P>0.05).All cases achieved bone healing after radiographic evaluations. The average bone healing time of the DLP group and RNP group was(7.40±1.68) M and (6.14±1.51) M respectively, with a significant difference (P <0.05). There's no complications such as loosening, bending and breaking of internal fixation, or Screw exit or incision of lag screw [Conclusion]Both treatment methods can achieve good curative effect for the treatment of nonunion of distal femur, and there is little difference of treatment effect between the two groups. The RNP group may have more advantages in perioperative period, duration of walking, bone healing time and full weight bearing time , while DLP may have more advantages in knee mobility.
