Abstract:【Abstract】 Objective To explore the application of perfusion and irrigation combined with negative pressure closed drainage (VSD) in the terrible triad of elbow joint. Methods A retrospective study was conducted of the 74 patients with terrible triad of the elbow joint who had been treated at our Hospital from January 2016 to December 2018 ,38 cases were placed perfusion and irrigation combined with VSD after operation. (Perfusion and drainage group), 36 cases were only placed in the drainage tube (simple drainage group). The perioperative period, reexamination, follow-up and imaging data of the two groups were compared. Results The patients in both groups completed the operation successfully, and there was no vascular and nerve injury during the operation. Compare the general data and Perioperative data ,The difference was not statistically significant(P>0.05). All patients were followed up for 10 to 45 months (mean 29.31±9.65months). Bone healing was achieved in both groups, but there was no significant difference in fracture healing time between the two groups(P>0.05). Compare the two groups at 6 months after operation and at the last follow-up differences in the Mayo elbow joint score, the range of elbow flexion and extension, the range of forearm rotation, And the last follow-up differences in the incidence of heterotopic ossification of elbow joint and the excellent and good rate of Mayo elbow joint score. The difference has statistically significant(P<0.05). Conclusion For the terrible triad of elbow joint after internal fixation surgery, placing perfusion and irrigation combined with VSD is beneficial to reduce the heterotopic ossification of the elbow joint, prevent adhesion, facilitate functional recovery, and satisfactory clinical results.
