Abstract:Objective To explore the distribution and prognosis of lower extremity DVT in the elderly patients with hip fractures, so as to provide the basis for clinical reasonable prevention and treatment. Methods The clinical data of 228 cases of hip fractures in our hospital from January 2016 to September 2019 were analyzed retrospectively,to evaluate the distribution and prognosis DVT before operation, the 1st week and 12 weeks after operation, and to analyze the related factors of DVT also. Results ① The total incidence of DVT was 28.68% (38 / 129) in patients with femoral neck fracture before operation, among which 75.67% (28 / 37) were patients with distal DVT and 31.57% (12 / 37) were patients with two or more branches. The incidence of proximal DVT was 13.51% (5 / 37), and that of mixed DVT was 10.81% (4 / 37).② The total incidence of DVT was 52.53% (52 / 99) in patients with intertrochanteric fracture before operation, which was significantly higher than that in femoral neck fracture (P < 0.001). The proportion of distal DVT and mixed DVT was 57.69% (30/52) and 7.69% (4/52), respectively. There was no significant difference between the two groups (P>0.05). The incidence of proximal DVT was 33.33% (18 / 52) ,significantly higher than that of femoral neck fracture (P < 0.05).③ There was no significant difference in gender, age, side, blood glucose and anesthesia between patients with DVT and those without DVT (P > 0.05). However, there were significant differences in BMI, waiting time before operation and operation mode in two groups (P < 0.05).④ 12 weeks after operation, ultrasound showed that thrombus was completely recanalized in patients with distal DVT, while with proximal DVT ,the recanalization rate was 60.71% (17 / 28), partial recanalization rate was 28.57% (8 / 28).and wiht mixed DVT, the recanalization rate was 36.37% (4 / 11), partial recanalization rate was 45.46% (5 / 11).Conclusion The incidence of lower extremity DVT was 28.68% - 52.53% in the elderly patients with hip fracture, of which the proportion of distal DVT was 57.69% - 75.67% and the proportion of proximal DVT was 13.51% - 33.33%. The postoperative recanalization rate of distal DVT is high, Anticoagulation treatment after operation is conducive to prevent the progression of thrombus, improve the recanalization rate, and avoid DVT related complications.
