Objective To investigate the effect of Z shape plasty for the treatment of soft tissue incarcerated of the complications in the transport distraction osteogenesis of tibia. Methods From July 2012 to November 2018 ,eight patients received the operation with the transport distraction osteogenesis of tibia who combined with soft tissue incarcerated.There were seven males and one females from 26—48 years (mean，38．3 years), including traffic accident injury in 5 cases, smashing injury in 1 cases, crush injury in 2 cases, with 3 patients suffering injury on the left side and 5 cases on the right side. The patients were treated by external fixator with the fractures of ankle joint following debridement，and after the legs were revascularized，vacuum sealing drainage was applied in emergency surgery, and then flap transplantatio was made to repair the defect of necrosis of skin and soft tissue on the second stage. On the third stage Ilizarov technique was take for the treatment of large segmental tibia defect . Z shape plasty was applied for the treatment of soft tissue incarcerated of the complications in the transport distraction osteogenesis of tibia.．Results All the flaps were survived, without vascular crisis, congestion and blisters.There was no reccurrence of the soft tissue incarcerated in next process of transportation of distraction osteogenesis . It turned out that the procedures were satisfactory. Conclusions It is a feasile way of Z shape plasty for the treatment of soft tissue incarcerated of the complications in the transport distraction osteogenesis of tibia,which has the advantages of simpler operation, less trauma, higher survival rate，and worthy of clinical application.
