Abstract:Abstract: [Objective] To introduce the surgical method of Suparpatellar approach for the treatment of tibia varus deformity. [Methods] A young patient with both tibia varus deformity in our department was enrolled in July 2016. The internal tibia was used to open the wedge osteotomy, and the autogenous iliac bone graft was used. The Suparpatellar approach was used to fix the orthopedic surgery. The bilateral tibia was 3D printed before operation, and the open osteotomy plane was located 3cm below the medial tibial plateau. The preoperative measurement determined the angle to be corrected. Referring to the planning data, the osteotomy and intramedullary nail were simulated on the 3D model before operation. The patient lies supine on the X-ray surgical bed, the axis of the tibia is parallel to the ground, and the right humerus is taken as an example. Osteotomy: the medial vertical incision of the proximal humerus is taken, about 6 cm long, and then cut in order, exposing the medial tibia, 3 cm below the medial platform. At the place where the saw is osteotomy, the pendulum causes the lateral cortex to stop, without interrupting the contralateral cortex, and the spreader is opened 10mm (1°=1mm principle). The wedge-shaped tibia was implanted into the medial osteotomy of the proximal tibia, and the medial open distraction was performed. The medial tibial osteotomy was satisfactory, and the intramedullary nail was fixed. [Results] This operation satisfies the patient's preoperative minimally invasive, beautiful, early motion, and increases the height requirements, and the intramedullary nail is firmly fixed. It can be walked down to the ground 48 hours after surgery, and bony callus was obvious in the osteotomy bone after 6 weeks postoperation,and the return visits are satisfactory in March, half a year, and nine months after the operation. [Conclusion] The medial open wedge osteotomy, the autogenous iliac bone grafting, and the Suparpatellar approach with intramedullary nail fixation technique meet the minimally invasive, strong fixation and early activity of the patient, and increase the various requirements of prolonging the limb. It is a kind of effective invasive surgery method for young people with knee varus orthopedics.
