Abstract:Abstract: [Objective] To compare the clinical effect of anatomical locking plate combined with cable and hook plate in the treatment of Neer type II distal clavicle fractures.[Methods] A retrospective analysis was made of 42 cases of distal clavicular fractures of Neer type II treated in our hospital from April 2011 to May 2018.According to different fixation methods,19 cases were treated with anatomical locking plate combined with cable internal fixation (the combined fixation group),23 cases were treated with hook plate internal fixation (the hook plate group).[Results] There were no early complications such as vascular injury, nerve injury and wound infection in both groups. There was no significant difference in operation time and bleeding volume between combined fixation group and hook plate group(P >0.05).42 cases were followed up for 12 to 20 months (17.52±4.66).Bone healing time in the combined fixation group was 10~12 weeks (11.86±1.80) and that in the hook plate group was 10~13 weeks (12.25±1.72).There was no significant difference between the two groups (P >0.05).The CCD of the two groups on the first day after operation was significantly lower than that of the preoperative group (P<0.05);the CCD of the combined fixation group on the first day and 12 months after operation was larger than that of the hook plate group (P<0.05).The late complications in 1 cases (5.26%) of the combined fixation group were significantly less than that in 8 cases (34.78%) of the hook plate group (P<0.05).There was significant difference in Constant shoulder function score between the two groups at 12 months follow up (P<0.05).[Conclusion] Compared with hook plate, the treatment of Neer type II distal clavicle fractures with anatomical locking plate combined with cable internal fixation has satisfactory curative effect, stable fixation, fewer complications and good recovery of shoulder joint function.
