Abstract:Cost-benefit analysis of the efficacy of artificial joint replacement//MA Teng-yang,PU Pei-dong,WANG-Zhi,QI Yu-xin,ZHOU Yu-zhuo,LI Ke,WANG Wei-shan,SHI Chen-hui.Orthopaedic Center, The First Affiliated Hospital,Medical college,Shihezi University,Shihezi 832000,China Abstract:[Objective] To provide a basis for rational control of medical expenses under the premise of ensuring the effect of surgery by providing a basis for the cost of hospitalization and its factors in the treatment of artificial joint replacement, and to provide a research basis for the relevance of medical insurance policy.[Methods] A retrospective investigation was conducted to collect data on the cost composition and postoperative efficacy of 849 patients who underwent “artificial joint replacement” in the Department of Orthopaedics, First Affiliated Hospital of Shihezi University Medical College from 2013 to 2018. And in combination with the relevant policies of medical insurance during the period, the impact factor analysis was carried out.[Results] In 2013-2018, the total cost of hospitalization for total hip arthroplasty in artificial joint replacement was 56,119.75 yuan, knee joint replacement was 55,518.69 yuan, and femoral head replacement was 45,784.22 yuan. The overall cost of different surgical categories was similar. Materials, medicines and surgery costs accounted for the largest proportion, 49.53%, 26.87% and 11.56%. The total hospitalization cost of patients undergoing artificial joint replacement increased year by year, and the material fee increased by an average of 28.16% per year. The proportion of medicines gradually decreased from 2015. There was no significant difference in the postoperative efficacy of artificial joint replacement in 2013-2018. The main factors affecting the total cost of hospitalization were the proportion of materials, the proportion of medicines, the number of hospital stays, and different types of surgery.[Conclusion] In the case of no significant difference in the therapeutic effect of artificial joint replacement, controlling the material cost of the artificial joint prosthesis and strictly controlling the clinical medication pointer, standardizing the hospitalization procedure is the key to reducing the hospitalization cost of artificial joint replacement surgery.
