Abstract:Abstract: [Objectives]To investigate the effects of holmium laser in PELD for lumbar disc herniation with posterior margin ossification.[Methods] From August 2016 to March 2018,55 patients with lumbar disc herniation associated with posterior margin ossification were included in this retrospective study. The clinical efficacy was evaluated using the Oswestry disability index (ODI) , visual analogue scale (VAS) , and modified MacNab criteria .55 cases of LDH with with posterior margin ossification approach in our institution were divided randomly into group A and group B. Patients in group A underwent the PELD combined application of holmium laser, while patients in group B underwent the regular PELD.Clinical data including the operation time, hospitalization and costs were recorded .Efficacy were assessed according to the visual analogue scale(VAS), Oswestry disability index(ODI) and modified MacNab criteria at the time of preoperative and postoperative 1 week, 1 month, 3 months and the last follow-up.[Results] A total of 55 cases completed minimum of 14±2 months. Threre were 21 cases in group A and 34 cases in group B. All cases underwent nerve decompression, nucleus pulposus and osteophyte excision under PELD .There was no significant difference in age, sex, lesion segment and protrusion type between the two groups (P>0.05). However postoperative VAS scores for back and leg pain, as well as the ODI, obviously improved in both groups when compared with those of preoperation (P<0.05). The postoperative VAS score for back pain、leg pain and Oswestry disability index(ODI) of group B and A with no significant difference (P>0.05) in a week after surgery.But after A month group A was lower than group B with a statistical significance (P<0.05). According to the MacNab criteria, the satisfactory rate was90 % and 82% in group A and B respectively, with no statistical significance (P>0.05). [Conclusions] PELD combined with holmium laser treatment with lumbar disc herniation with posterior margin ossification.can obtain better effects.
