Abstract:[Objective] To compare the surgical outcomes for femoral subtrochanteric fracture using proximal femoral nail antirotation and locking compression plate versus intramedullary nail. [Methods] A retrospective study was conducted on 46 patients with subtrochanteric fracture who underwent surgical treatment from January 2013 to December 2017. The study group (27 patients) was patients who underwent surgical treatment using proximal femoral nail antirotation and locking compression plate. The control group (19 patients) was patients who underwent surgical treatment using intramedullary nail. The data of patients from study group: 15 males and 12 females aged 29~90 (65±16.86) years. The data of patients from control group: 7 males and 12 females aged 49~86 (72±10.23) years. The operation time, blood transfusion rate and the Harris hip score were compared between two groups. [Results] The operation time of the study group was 163±27.10min. The operation time of the control group was 80±5.54min. The study group had significantly longer operation time (P<0.01). The blood transfusion rate of the study group was 74.07% (20/27). The blood transfusion rate of the control group was 36.84% (7/19). The study group had significantly higher blood transfusion rate (P<0.05). The Harris hip score of the study group was 91±4.73. The Harris hip score of the control group was 88±3.79. The study group had significantly higher Harris hip score (P<0.05). The fracture healing time of the study group was 3.89±0.80months. The fracture healing time of the control group was 4.47±0.77months. The study group had significantly shorter fracture healing time (P<0.05). All operative incision were well healed. No complication like fracture displacement, fracture nonunion and fixation failure occurred. [Conclusion] The surgery using proximal femoral nail antirotation and locking compression plate is a safe and effective method to treat comminuted subtrochanteric fracture. It can achieve better hip joint function and shorter fracture healing time. But meanwhile it had longer operation time and higher blood transfusion rate.
