Objective To analysis the clinical effect of four corner fusion with locked micro-titanium plate combined with scaphoid resection for the treatment of scaphoid nonunion advanced collapse wrist (SNAC wrist). Methods We treated 5 patients with SNAC wrist who underwent scaphoid excision with plate four corner fusion using locked micro-titanium plate from June 2011 to September 2016. According to Waston staging criteria, all cases were rated as stageⅢ SNAC. The examination included wrist pain, hand grip strength, wrist movement. Results Primary healing of incision was achieved,and no complication such as bone nonunion and wound infection and skin necrosis occurred. The mean follow-up was 26 months(range,10-38 months). All five wrists were absolutely free of pain,while one of them had occasional pain on heavy work in long time.Grip strength had improved significantly in all patients. The wrist pain relieved obviously .The mean range of the wrist reached over 50% of the healthy side. Conclusion Four corner fusion with locked micro-titanium plate combined with scaphoid resection is an effective and safe method of the treatment for scaphoid nonunion advanced collapse(SNAC) wrist,which could provide obvious pain relief and with reliable fixation and high fusion rate.
