Abstract:Objective To explore the method of repairing and reconstructing Achilles tendon defect and summarize the clinical efficacy of repairing Achilles tendon with hallux long flexor tendon. Methods From January 2008 to January 2016, 17 patients with Kawanda Ⅱ-Ⅳ Achilles tendon defect were retrospectively analyzed. Of these, there were 8 males and 9 females, aged from 30 to 60(48.28±8.56) years.All the patients suffered from unilateral Achilles tendon repture which was confirmed to have a 2cm-6.5cm Achilles tendon defect during the surgery. The length of the hallux long flexor tendon was 15-38cm. After Achilles tendon repair,the time of disconnection,recovery time,ankle muscle strength,range of movement and operative complications were observed, and the curative efficacy was evaluated by Leppilahti Achilles tendon repair score. Results 17 cases of this group were followed up for 1-8(1.73±1.31) years. The removal time was 14-30(19.62±3.77) days,the recovery time was 3-5(3.91±0.39) months,the ankle muscle strength was above grade 4,the shape recovered well, and the thumb flexion and extension function was not affected. There were no cases of persistent non-healing of Achilles tendon exposure and no re-rupture of Achilles tendon.The Leppilahti score of Achilles tendon repair was 78.13-95.82(mean 90.02±5.10),of which 13 cases were excellent and 4cases were good. Conclusion The hallux long flexor tendon has enough length and good quality. The repair and reconstruction of Kawande II-IV type Achilles tendon defect is of high quality, the function and shape are good, and the complications are few, this operation method is worth popularizing.
